Requirements for Service Providers

  In order to be eligible for employment at Divine Choice Home Health Agency, “LLC.” You must submit all of the required   documents below, which are mandatory before you can begin to render services under our Agency.

  ·         Complete Employment Application

  ·         Complete Employment Eligibility Verification Form I-9. (Non-citizens must provide proof of employment     authorization.)
  ·         Submit copies of the following documents:
  o    Social Security Card
  o    Driver’s License- __________
  o    Vehicle Registration- __________
  o    Vehicle Insurance- __________

  ·         Submit proof of High School Diploma or equivalent

  ·         Submit proof of at least One Year Experience working in a medical, psychiatric, nursing or child care setting, or in working with recipients having developmental disabilities. College, Vocational or Technical Training from an accredited institution can substitute at the rate of 30 semester, 45 quarter or 720 classroom hours for the                            required experience.
  ·         Complete Background Screening. Updated every 5 years. (See procedures attached)
  ·         Complete Recent Local Background Screening.
  ·        Complete the following required trainings or submit proof of certification:

  o    CPR- ___________
  o    First Aid- ___________
  o    Infection Control- ___________
        HIV/AIDS- ___________
  o    Zero Tolerance (every 3 years) ___________ 
  o    HIPAA- ___________
  o    Affidavit of Good Moral Character ____________

ZERO TOLERANCE POLICY REGARDING ABUSE OR NEGLECT OF INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES

Purpose:
Divine Choice Home Health Agency “LLC” is committed to promoting and
assuring the safety and security of our clients. Therefore, Divine Choice has adopted a “zero
tolerance” policy regarding abuse or neglect of individuals with developmental disabilities, as well as failure to report suspected abuse or neglect.

Definition:
Abuse includes physical abuse, neglect, financial abuse, abandonment, isolation,
abduction, or other treatment which results in physical harm, pain or mental suffering or deprivation, by a person providing care and supervision, of goods and services which are necessary to avoid physical harm or mental suffering.

Physical Abuse: The use or attempt to use force on the person of another, unreasonable physical constraint, sexual abuse, or prolonged or continual deprivation of food and water. It also means the use of isolation, physical or chemical restraint, or psychotropic medications without medical authorization, for the purpose of punishment.

Mental Suffering: Fear, agitation, confusion, severe depression or other forms of emotional distress that is brought about by threats, harassment or other forms of intimidating behavior.

Neglect: Negligent failure of a person(s) having care or custody of an individual to exercise a reasonable degree of care including, but not limited to, a failure to assist in personal hygiene and the provision of food, clothing, and shelter, or failure to provide medical care or protection from health and safety hazards.

Responsibility:
All caregivers providing services to Divine Choice clients.

Policy:

Client abuse committed by Divine Choice employees will not be tolerated. All such abuse or allegations of such abuse shall be reported immediately to the proper authorities, Failure to report shall not be tolerated.

Divine Choice Employees: Any abuse or allegations of abuse involving a Divine Choice employee shall be reported to the appropriate governmental authorities. Any Divine Choice employee found to have engaged in abuse against a client shall be subject to severe discipline, up to and including immediate termination. Any Divine Choice employee who fails to report abuse or allegations of abuse shall be subject to disciplinary action, up to and including immediate termination.

Procedures:
Notice upon Hire: Divine Choice and all of Divine Choice Caregivers serving Divine Choice clients shall ensure that their employees are fully informed upon hire regarding this Policy and the mandatory abuse and neglect reporting laws.

Divine Choice Annual Notice: Divine Choice shall notify its employees, of this Policy on an annual basis.

Divine Choice Posting of this Policy on its Website: Divine Choice shall promptly post and maintain this Policy on its website, no later than November 1, 2015.

Immediate Action Required: If Divine Choice staff becomes aware of client abuse or neglect, that agency and/or person shall take immediate action, to the extent permitted by law, to ensure the health and safety of the affected client and all other clients receiving services and supports from Divine Choice.

Inconsistencies:
If any inconsistency exists between this Policy and the reporting laws, the provisions in the reporting laws shall prevail.

This policy was reviewed and approved by the Chief Executive Officer on October 29, 2015 and is effective immediately.
